Job Description:
LPN or BSW Case Manager - Big Cabin, OK - Big Cabin, OK
Mays Plus, Inc. is seeking an LPN with current license without
restriction or a Social Worker with their bachelors. Must have been
licensed for at least a year with a year of geriatric experience
(ex. home health or nursing home). Must work well without
supervision. Computer experience with typing skills required. This
is a full time position with competitive pay.ADvantage Case Manager
Job Duties Include:--Initiating and monitoring service plan/goals
and performing resource referral as appropriate to meet the needs
of ADvantage Program Members. Continually documenting Member
response to plan and interventions. Acting as Members
staff/community liaison. Skilled Nurse and supervisory visits
needed. Good communication skills. Fast paced office.
Monday-Friday, 8-5, no weekend, no on call, and no holidays.Please
