The Pediatric Care Manager offers case management/care coordination to pediatricpatients and their families with complex health care needs andcontributes to the five foundational services in the Health Access Network:facilitating Members' access to all levels of care utilizing casemanagement/care coordination services, telemedicine services, electronic healthrecords, improved access to specialty care services and expanded qualityimprovement strategiesEnvironmental Hazards: Possible exposure to hazardous material, hot and cold, blood borne pathogens.Physical Requirements: Ability to lift and carry 8-10 pounds, stoop, reach, stand, walk, finger,grasp, feel, talk, hear, see, and perform repetitive motions with or withoutreasonable accommodations.Special Instructions to ApplicantsIMPORTANT: Please ensure all employment history is correct and complete, including employer name/position title, dates of employment, salary, supervisor name/contact information, work experience and reason for leaving. An Edit button is available on the far right which allows you to add/change information.Education & ExperiencePosition Qualifications:Required:-Associate's degree in Nursing plus 2 years nursingexperience OR, Bachelor's degree in Nursing plus 1 year of experience asan RN OR, Master's degree in Nursing OR, Master degree in Social Workplus 1 year of experience as an LCSW. -Current & active RN, or LCSW licensure issued by Oklahoma State Boards ofNursing or Social Work. -Valid Driver License. -Case management/outpatient clinic experience. -knowledge of or experience in pediatrics -On Dec. 7, 2021, a federal judge temporarily blocked the implementation of thefederal contractor vaccination mandate and its associated guidance. Followingthe injunction, Oklahoma State University has, consistent with applicable law,paused the vaccination mandate.Skills,Proficiencies, Knowledge:Knowledge and skills in chronic disease management. Knowledge of national,state and local health and social service provider agencies; exceptionalinterpersonal communication skills - to include writing/documentation skills.Critical thinking skills and sound judgment; exhibit initiative and ability towork independently and within a team; ability to meet deadlines and prioritizeworkload; exhibit cultural sensitivity and the ability to work with diversepatient population and professional staff, faculty and medical students withinacademic health care setting. Computer skills.Preferred:-A minimum of 3 years clinical practice experience -Case management certification (or eligible to obtain).
