    Responsibilities Serve as a patient navigator throughout the continuum of care.
    • Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
    • Coordinate efforts for treatment and promote quality improvement and educational efforts.
    • Compile data, track outcomes, and make recommendations for process improvement.
    • Provide referrals/support for follow-up care for patients who are uninsured or underinsured.
      Requirements Licensure / Certification / Registration:
      • Licensed Registered Nurse obtained prior to hire date or job transfer date required.
      • Registered Nurse credentialed from the Oklahoma Board of Nursing required.
      • BLS Provider preferred. American Heart Association or American Red Cross accepted.Education:
        • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
          Additional Preferences The overall responsibility of the RN Care Manager, or RN Patient Navigator, is to provide clinical patients with the support they need to successfully continue their care plans once discharged from the hospital. - Some of the tasks include:
          • Motivational interviewing and education to promote patient progression toward their health goals.
          • Develop care plans and work with the providers to field questions regarding patient who are preparing to be discharge from the hospital.
          • Determine risk stratification - what factors does the patient have and what risk score do you give that patient.
          • Medication reconciliation
          • Prepare discharged patient for follow up appointments with the provider in clinic - -#SponsoredJobOK
