Social Worker -Substance Use Disorder-Behavioral Health
Interdisciplinary Program - Tulsa, Oklahoma
Duties
- Develops assessment of the Veteran in collaboration with the
BHIP team, the Veteran, family members and significant
others.
- Utilizes assessment tools to highlight the Veteran's strengths,
limitations, and internal/external supports and service needs in
order to optimize the Veteran's functional status, formulate a
diagnosis, and lay the foundation for the treatment plan, if
necessary.
- Demonstrates an understanding of the range of treatment and
skills needed for substance abuse therapy, for all adult patients,
including those with co-occurring disorders.
- Understands the factors and causes of substance abuse, as well
as the problems related to these disorders.
- Integrates the treatment of SUD and concomitant
disorders.
- Accurately diagnoses the different types of SUD and psychiatric
disorders.
- Demonstrates an understanding of the range of treatment and
skills needed for family counseling.
- Conducts reassessment at intervals specified in program
policies.
- Is responsible for developing the treatment plan and setting
achievable and measurable treatment goals with the
Veteran/family.
- Is responsible for educating the Veteran and/or caregiver of
the available services and assisting them in establishing the
appropriate referrals based on the Veteran's preference or that of
his surrogate decision-maker.
- Collaborates with other service providers in reassessing the
Veteran's needs.
- Educates and encourages the Veteran/family to advocate in their
own behalf, thus fostering a sense of independence and
empowerment.
- Provide crisis intervention, stabilization, and follow-up for
Veterans and significant others.
- Works with other disciplines to coordinate the most effective
care that can be provided.
- Reviews the progress notes from the other providers to
accurately determine the strengths and limitations of each Veteran
being referred for SUD services.
- Provides the Veterans and their caregivers with ongoing
supportive counseling.
