Job Summary:

The Health Risk Assessment Coordinator is responsible for ensuring completion of a health risk survey for all newly enrolled Medicare and Marketplace members. Responsibilities include identifying health risks scores, generating reports and distributing data to the responsible departments.

Responsible for servicing the health plan needs of members and providing information and education on benefits and services to Medicare members.

Key Responsibilities:

• Conduct and organize data pertaining the health risk assessments survey. Identifying potential health risks for Medicare and the Marketplace membership.
• Responsible for contacting members and distributing HRA risk scores to the various departments for all newly enrollment Medicare and Marketplace members.
• Run Wellsource reports to maintain the HRA database. Perform and distribute monthly HRA audits and reports.
• Schedule and conduct monthly benefit meetings to enhance the understanding of benefits for members. Conduct annual benefit meetings to educate members about benefit changes for the upcoming year.
• Assist Member Advocates at the Senior Center to answer phones and walk-in questions. This position may involve dealing with members who are disgruntled or upset.
• Handles additional responsibilities as required. Qualifications:
  • Highly organized and attentive to detail.
  • Strong oral and written communication and interpersonal skills.
  • Capable of managing multiple tasks at one time.
  • Skilled in utilizing Microsoft Office Suite and database software.
  • Successful completion of Health Care Sanctions background check. Education/Experience
    • High School diploma or equivalent.
    • Related customer service experience in managed care, preferred.
    • Knowledge of Medicare programs, preferred.
    • Ability to lead and coordinate large group discussions, preferred.
