Job Description We are looking to add a fellowship trained
Geriatric Physician BC in either Internal Medicine or Family
Practice to our multidisciplinary team. The practice has 2
fellowship trained geriatric physicians, 2 Advanced Practice
Providers, a full-time pharmacist, a social worker, and a community
case manager. Practice hours are Monday-Friday, 8 am – 5 pm. The
practice offers a full range of primary care for men and women aged
60 and older. Our providers take the time to listen to the
patient’s health concerns and create a treatment plan to meet their
health needs. Our physicians have special training in treating
seniors and employ the most current information, treatments,
medications and practices for disease prevention and diagnosis. We
