Job Description:
Ascension is seeking a dedicated RN Registered Nurse Patient
Navigator to join our faith-based healthcare organization. In this
role, you will guide patients through the complexities of the
healthcare system, ensuring seamless, coordinated care and
providing emotional and practical support. By working closely with
healthcare providers, you will help patients understand their
conditions, treatment options, and the necessary steps they need to
take within our healthcare network. This position is pivotal in
empowering our patients and improving health outcomes by delivering
personalized care in line with our mission to innovate and
transform healthcare. Educate and support patients and their
families about health conditions and treatment plans. Coordinate
with healthcare professionals across the system to ensure a
seamless patient experience. Assist in scheduling appointments,
organizing transportation, and facilitating communication between
different service providers. Monitor patient progress and advocate
for their needs within the healthcare system. Document patient
interactions and interventions appropriately in medical records.
Strong interpersonal and communication skills Ability to handle
sensitive information confidentially Knowledge of medical
terminology and healthcare system operations Proficiency in EMR
(Electronic Medical Record) software Strong organizational and
problem-solving skills Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Valid RN license Certification in
patient navigation is preferable, but not mandatory
