We Are Hiring Ascension St. John Medical Group proudly serves the people of Owasso, Oklahoma and the surrounding communities. We are seeking a Licensed Clinical Social Worker for our Primary Care Owasso. As part of our commitment to delivering personalized, compassionate care, Ascension St. John Medical Group is expanding to provide patients with social staff who will investigate, assess and plan interventions to help patients cope with social, emotional, economic, behavioral, and environmental challenges. In this role you will develop and maintain working relationships with community health, welfare and social agencies to assist the client with his/her needs. In doing so, you will be able to assist clients/families with obtaining community assistance by referring them to proper resources. Schedule: Full-Time, Days Responsibilities: * Conducts patient assessments and establishes therapy treatment goals. * Administers individual and group therapy. Maintains thorough and confidential records of observations. * Evaluates progress and effectiveness of therapy interventions. * Provides patient with information regarding resources and therapeutic activities in the community. * Works directly with the patient, family, physician and community providers to ensure active and collaborate care from the initiation of treatment to the time of discharge. Licenses/Certifications/Registration: Required Credential(s): * One or more of the following: * Counselor credentialed from the Oklahoma State Board of Behavioral Health. * Social Worker specializing in Clinical Practice credentialed from the Oklahoma State Board of Licensed Social Workers. * Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) required. Preferred Credential(s): * BLS Provider credentialed from the American Heart Association (AHA). * Clinical Counselor.
